Before (front and side views): This patient is barely forty but was bothered by the dark circles and puffiness under her eyes for many years. We discussed non-invasive treatment with fillers versus surgery and the risks, benefits, and expectations of each approach.
After (front and side views): These photos show the patient in front and side profile three months after transconjunctival lower laser blepharoplasty with fat transposition into the tear trough. Doing a lower laser blepharoplasty alone would not correct the hollow indentation causing the dark circles between the bulging eyelid fat pad and her cheek. No incisions were made on the skin. While some plastic surgeons will perform this surgery using a skin approach, Dr. Enzer prefers the transconjunctival approach (inside the lid) to avoid complications of the eyelid getting pulled down or a visible scar. Often, Dr. Enzer will recommend a concurrent carbon dioxide (CO2) skin laser resurfacing to tighten the skin, but it was not done for this patient. She and Dr. Enzer were thrilled with her results.
