Please download the registration packet and bring the completed forms with you to your appointment. You may also send the completed forms to our office ahead of time. Additionally, please bring your insurance cards and a photo ID to your appointment. If you are being seen for an eye-related issue and you wear contact lenses, please bring your contact lens case and glasses.
Cosmetic Patients: Please bring past photos from high school, mid 20’s, mid 30’s and so forth for Dr. Enzer/Ashley Wright, FNP to observe your aging progression.
